This is the third form of the SmartOB Version 3
forms. As with Version 2, this form as well as all the others has been designed to perform optimally with a Tablet computer
using just the pen as the input device in most instances. This facilitates
workflow during the prenatal visit. Version 3 Form 3 differs significantly from
that in Version 2. More data is carried over from Form 1, including Age,
Allergies, G, T, P, A, L and BMI. Prenatal Issues are brought to the top of the
form. The Prenatal Lab area has been moved and expanded to its own page, Form 5.
The Ultrasound section is greatly expanded. Most of the form is self-explanatory. The details to the right are for
clarification of the items that might need some added explanation.

The
medications list has been moved to this form. In addition, dosage and
frequency has been added and can automatically be inserted by the
Progress Notes form on SmartOB Form 4. Insulin dosages can also be seen
and entered as am & pm NPH/Reg. If that is not how the patient
administers her Insulin, then the exact dosage can be entered on the
Progress Notes on OB Form 4. |
When this form
loads for the first time during a session, a Warning box pops up
reminding the user to document any missing data based on the EGA as
calculated from the Final EDD, the computer date and the Preferences
set by the user if not already warned. As with the other forms, pop-up boxes facilitate data
entry.
The Name, Age, Number,
Allergies, G, T, P, A, L, Pre-Gravid Weight and BMI are carried over from Form1. LMP and
EDD
are carried over from Form1 as well but none of the
calculations are based on those dates.
The Final EDD is
set by you. Every calculation performed by these forms is based only on
that date so no calculations are performed until that field is filled
in.
Up to 8
problems, concerns or issues can be documented in the Prenatal Issues
fields. If the Issue is selected from the drop-down list, the ICD code
is automatically filled in for you.
When you enter
the visit Date, the EGA is automatically calculated and filled in for you.
If the B/P > 140
systolic or 90 diastolic there is a warning. Thresholds over baseline
can also be set on Preferences.
Weight gain can
also be monitored by the forms and warnings provided.
When you enter
the date and EGA of the ultrasound, the EDD by ultrasound is
automatically calculated for you.
The rest of the
fields per visit are drop-down boxes that may be used to increase data
entry speed.

This form pops up when the Pediatrician field is entered on Form
3. If the field is already selected, you may access this form by
double-clicking in the field. You may create a list of up to 30
Pediatricians or Pediatric Groups. This list, as with others, should be
saved in your template form so that it is available for every new
patient. |

This form pops up when the Notes field is entered on Form
3 & 3 (2). It allows you to quickly and
effortlessly build >90% of your Notes without having to use a
keyboard. The default button is
Finish, so hitting the Enter key will close this form.
Clicking the Add to Notes button
will add the selected notes from the list to the Notes. Clicking
the Delete from Notes button will delete the highlighted
selection from the notes wherever it is located. Copy Previous
will copy the previous notes into the Notes.
You may also type and edit directly in
the Notes field.
When you're done, just select Finish.

This form appears when an empty INITIALS
field is selected on Form 3 and Form 3 (2). It allows for entering
Initials without a keyboard and provides management of the Initials on the Forms.
The Initials are limited to 3 letters.
Once you enter the Initials of those who will be
typically using the forms, you may save them as part of the template
forms so you don't have to keep entering them with every new patient. |
The Prenatal Issues list is where the user can document
the 8 most concerning diagnoses for the current pregnancy. Each field in the
Prenatal Issues list at the top
contains a drop-down list of some high risk diagnoses that can be pulled up and
modified
as desired. If you use the diagnosis as worded, the ICD code is automatically
supplied to the right.
Lastly, Form 3 (2) looks identical to the above
form and all fields are carried over from this form to 3 (2). Form 3 (2) is a
continuation form that is available should the patient require more than the 14
visits available on Form 3. As such, the only fields available for data entry
are the visit and ultrasound fields.
