The patient's chart number auto-repeats on all subsequent forms. The patient's name also auto-repeats on all subsequent forms. The patient's age is at the EDD and is calculated for you based on the patient's DOB and her Final EDD that you enter manually on Form 3. When you enter an LMP, the EDD is calculated for you automatically on Form 3. If you put a ?, the calculated EDD on Form 3 is blank. All calculations are based on the Final EDD entered manually on Form 3. The medical history is set with -'s for the patient and family initially to speed entry on new OB's but they can be set to blank if desired. If Latex is listed as an allergy on Form 1, then Latex Allergy appears at the top of Form 3. The forms compare B/P on subsequent visits to the initial B/P and will warn if the B/P exceeds thresholds set by the user. The Age, G(gravida), T(term), P(preterm), A(pre-viable), L(living), Pre-gravid weight and BMI are also repeated on Form 3. Allergies is set to NKA by default but may be set to blank if desired. Again, to speed along data entry on new OB's, by default, the physical exam is checked as all normal but may be set to blank if desired. Also, the bony pelvis is listed as adequate by default, but this can be left blank initially if desired.
