SmartOB^® Prenatal Forms are a set of intelligent electronic prenatal forms that run in the Microsoft Excel environment. You may download the forms for free and may test them indefinitely prior to your decision to purchase an annual license fee. The license allows ONE (1) purchaser to copy and save an unlimited number of any version of the SmartOB^® Prenatal Forms and any future upgrades for one year. The user will receive a warning starting 30 days before the license expiration to renew the license. Once the license expires, all existing versions of the prenatal records will continue to load and run forever, thus no data or functionality will be lost, but no additions or changes to the forms will be possible.

Annual License fee is $29.95 for unlimited use by the purchaser for one year.
